BULK or COMMERCIAL ORDERS

PLEASE READ FIRST!
  • NCDOT requires both a MAILING address (PO Box, etc) AND a STREET address. If the Street Address is also the postal delivery address, indicate by "SAME" or "SAME AS STREET ADDRESS".
  • The maps are packaged 25 to a bundle and 200 per case. Quantities in multiples of 25 or cases will be faster to mail or ship out. However, hand counts or any reasonable quantity will be shipped, subject to availability.
  • Quantities over 25 will go UPS COD (COD for the freight charges) and smaller quantities by mail.
  • If the Street and Mailing Address have different zip codes, please give the DELIVERY zip code. This will be the Street Address for UPS shipments.
  • If you are using this form for large quantity (COD freight) orders, use the comment section to list desired quantities.
  • If your business currently has a web page and/or email address, please include them in the contact information.
